A Functional Safety Audit of the FSM procedures for a gas distribution company.

Stage 4 Functional Safety Assessment of selected oil refinery process units.

Stage 4 Functional Safety Assessment (FSA 4) at an) Liquified Natural Gas (LNG) site. The site is a storage facility, with the storage being at 2.5 to 5 barg. The SIFs are low temperature SIFs, designed to protect carbon steel pipework downstream of the two vapourisers, with one SIF per vapouriser.

Project to perform a Prior Use assessment for the Phoenix Contact Relay (PCR) as installed at an oil storage site.

Project to perform a Prior Use assessment on an Intrinsically Safe Trip Amplifier as installed at an oil storage site.

A project to deliver a Human Factors Analysis (HFA) of selected pipeline receipt tasks at a pipeline terminal. The purpose of this study was to evaluate the appropriateness of the use of the (industry standard) figure of 0.01 applied to human reliability or probability of failure within a recent LOPA Report. Through application of the HEART+ (Human Error Assessment and Reduction Technique) a study on the Human Factors (HF) aspects associated with the pipeline receipts tasks was undertaken. This provided a figure for human error; the assessed nominal likelihood of failure, which could then be directly used in the LOPA study in place of the generic data previously used.

Project to carry out a Stage 1 Functional Safety Assessment (FSA 1). The assessment assessed the compliance of the duty holder and project vendors’ functional safety management systems in relation to the delivery of a major new installation.

Project to carry out a Stage 2 Functional Safety Assessment (FSA 2). The assessment assessed the compliance of the duty holder and project vendors’ functional safety management systems in relation to the delivery of a major new installation.

Project to develop overall safety plans (as per IEC 61511) for a major new installation.

Project to carry out SIL Verification and confirmation of PFD calculations for x22 SIF's.

An update to an earlier SIL verification and PFD calculation exercise.

For an oil storage facility, carrying out an exercise to confirm that the SIL requirement was correct (under review changed from SIL 1 to SIL 2), identify a replacement for non-compliant contractor and confirm the compliance of a non-certified galvanic isolator by way of a prior use analysis.

Providing a high-level review of a downstream oil industry client's Cyber Security Risk Management procedures.

3-year update to earlier Proven-In-Use study of final element (valve) used in tank farm.
